1. I will follow-up and document Clients regarding
his/her current situation and evaluate why he or she is still unemployed and record information into client files.
2. I will conduct an in person client interviews
and record information assessing the reasons why clients are requesting the services
they need such as job referral, lowering utility expenses and educational services so I can give information to them to see
if they qualify for those services.
3. I will answer 60 incoming phone calls and provide
clients with information regarding the services offered by my agency such as food, clothing, hygiene, shelter, and other referral
services in our department.
4. I will file and organize 20 client charts in
the correct order for a period of 12 weeks.
5. I will hold one individual grief counseling session
per week for 12 weeks addressing issues such as acceptance and coping skills with family members of the deceased.
6. I will conduct six group counseling sessions
on grief and loss with the curriculum assigned by my supervisor this is to be completed within 14 weeks.
7. I will facilitate three workshops for Long Beach
target area at GRiP conferences three times this semester topics will include violence prevention skills, alternative to gang
affiliation and anti-gang curriculum.
8. I will contact and arrange for 10 site visits
to local agencies which Long Beach Grip could be referring its youth and families to. This is to evaluate the programs for
referral.
9. I will add five pages of information to Long
Beach Grip website recurrent regarding 15 agencies or activities to which we could refer as means of intervention or as an
alternative to gang affiliation.
10. I
will complete ten intakes for drug and alcohol abuse for entry into her program using applicable questionnaires that address
drug of choice, past treatment, unemployment, and homelessness.
11. I will record daily information on seven clients
and note daily progress towards assigned individual goals including group compliance, attendance, and completing individual counseling sessions.
12. I will organize and file seven client charts
in the correct order for three weeks and record information and I will note information on the outcomes for our behavior study.
13. I will facilitate four chemical dependence groups
addressing the following: drugs of abuse and codependency, alcohol, meth and crack cocaine. We will discuss triggers, how
we obtain drugs, what to do in a tempting situation.
14. I will answer five incoming phone calls to my
agency and refer callers to appropriate department within my agency such as drug and alcohol prevention department.
15. I will complete four intake assessments. I will
assess the clients drug and alcohol use and documentf previous prevention treatment attempts.
16. I will complete five one-on-one orientation
counseling sessions with clients. Counseling topics that will be discussed will be on their treatment plan and concerns they
might have about the program.
17. I will complete six assessments regarding past
treatment. Such topics will include drug and alcohol usage, living situation, prior treatment programs, and program commitment.
18. I will administer 10 urinalysis tests to clients
and record results in their charts.
19. I will assist 10 clients to their doctors visits
and will help transport them to these appointments.
20. I will assist three clients with their meals
and help them to feed themselves.
21. I will assist in 10 recreation activities for
the senior hospital including bingo,
games, and doing puzzles, one per week for ten weeks.
22. I will
complete 5 intakes using applicable questionnaires –
the AS1 (alcohol severity index) and note drug and alcohol usage, legal history, and prior treatment efforts.
23. I
will counsel three clients and provide them with three interventions to prevent relapse and to deal with a desire for alcohol and drugs, low self worth, and denial and anger issues.
24. I will record weekly information in files on
three clients for nine weeks and note progress toward individual goals related to legal issues, work issues, family issues
and progress in a 12 step program.
25. I will
complete 10 intakes to assess potential client’s
qualifications to be admitted to the program. The qualifications will include income, drug abuse, past treatment, and willingness
to adhere to program guidelines.
26. I will facilitate 10 group counseling session
topics to be addressed will be triggers, dealing with emotion, anger expression and relapse prevention.
27. I
will answer 10 incoming calls and refer clients to appropriate outside agencies to meet their needs. These agencies willing
include homeless shelters, food banks, clothing, and psychological treatment.
28. I
will hold two group counseling meetings per week for 10 weeks to discuss matters involving relapse prevention, anger management, men's issue, aftercare, grief, stress management,
parenting issues, life skills, and reentry into society.
29. I will organize and file three client charts
and documents for a period of 12 weeks noting information on drug of choice, length of sobriety, criminal records, medications
being used, and progress within the organizational program.
30. I
will teach approved curriculum to six clients on substance abuse, relapse prevention, triggers, and anger management. I will
do this once per week for six weeks.
31. I will co-lead three group counseling sessions
on relapse prevention addressing triggers, personal association choices and making appropriate choices.
32. I
will record daily information on at least for clients noting daily progress towards treatment goals and put in their file.
33. I
will answer incoming calls and referred 10 clients to the appropriate services outside our agency I will refer to resources
including social services, social assistance agency, food banks, and job assistance programs.
34. I
will document 10 client case files in order to track client's progress on not smoking drugs or using alcohol. I will interview weekly and record random drug screening results and compare to their answers during interview
process.
35. I will
administer the drug and alcohol severity index and score the results for 10 clients, record in client’s records and discuss the results with my supervisor.
36. I
will contact 10 Bible study groups to discuss the ways the Bible helps to keep one living a sober life. The study groups will
have a question and answer session to assess the materials and my group leadership.
37. I
will look totally 10 group counseling sessions on drug and alcohol education. Topics will include maintaining sobriety, avoiding
triggers, basic work ethic, domestic abuse and the physical consequences of their behaviors.
38. I
will co-lead 10 life skills groups under the direction of my supervisor. The skills group will cover the following topics:
hygiene, food shopping, food preparation, and money management.
39. I
will complete a minimum of four intakes assessing the clients drug and alcohol use and their past treatment attempts.
40. I
will conduct a minimum of three orientation groups to teach clients important rules and information about my agency's program.
41. I will co-facilitate three family support groups
discussing issues as codependency, domestic violence, and proper parenting and supporting the addicted family member.
42. I
will complete six new intakes using applicable instruments that questionnaires on anger management, domestic violence, parenting,
and drug and alcohol use.
43. I
will answer calls and refer 10 clients for appropriate services outside my agency. I will refer to services including homeless assistance, employment counseling, and
mental health services.
44. I will facilitate six addiction treatment related
group meetings. The issues to be discussed will include living drug-free and how to maintain sobriety in a world filled with
temptations and drugs.
45. I
will conduct three orientation roots to teach clients important rules and information about Long Beach City College's Women's and Men's Resource Center and how we can assist them in their needs both educational
and personal.
46. I
will properly refer seven clients to the new services as necessary and appropriate for their individual needs such as for
food, housing, and health services.
47. I will teach to team clients how to access specific
resources in the community on their own for their individual needs this will include computer searches, telephone searches
and referral organizations. These services will deal with issues of domestic violence, drug and alcohol abuse, and family
health and psychological services.
48. I
will assist 10 people in filling out applications for college programs and benefits. These programs and benefits include financial
aid. EOPS, Scholarships, and Disabled Resources Services.
49. I will be five individual intake interviews
in-person, with those who are elderly and/or physically impaired and refer to the proper agency services within the community.
50. I
will conduct five orientation groups teach clients important rules and information about Long Beach city College is women's
and men's resource Center and how we can assist them in using our computer system and data base of referrals.

will properly refer seven clients to community services as necessary and appropriate for their individual needs such as for
food, housing, drug and alcohol services, and health services.

will co-facilitate five wellness groups addressing their compliance with medication and how to remember to take medication
at the recommended time. We will discuss why the medication is necessary for their future health.

will complete five individual counseling sessions with clients. The sessions will be regarding their drug and alcohol history,
how to avoid domestic violence situations, drug and out all use triggers, and how to learn to associate and make friends with non-drug abusing individuals.

will assist 10 needy teens on subject
areas of math, English, social studies. I will do this three times per week for 12 weeks.
55. I
will develop and implement a monthly calendar and set up activity programs for four months. The activities will include painting.
Role-playing feelings and responses , expressing emotion through art activities. I will complete these activities three times a week for 12 weeks.
56. I
will facilitate 10 group counseling sessions with teens. Topics will include peer pressure and how to avoid joining a gang,
avoiding peer pressure to use drugs and alcohol, Working on parental problems,
and how to handle violence in their homes and what to do if a threat is at hand.
57. I
will review 100 grievances and enter the consequences into the client database and record outcomes and if the consequences were completed in a timely and satisfactory manner. This will be reviewed by the director
of my program.
58. I
will head a new client orientation group every week and explain the rules of the facility and how new client needs to behave
and what will be expected of them to successfully complete the program. The consequences of non-compliance and rule breaking.
59. I
will review 10 daily logs including the residential sober living log and escort log. I will take the list of 180 clients and
compare them with the 25 group counseling progress notes and sign in sheets to assess if clients have been attending as instructed
by comparing group logs and escort logs.
60. I
will co-lead 10 group counseling sessions. Topics will include connecting with our higher power, attendance at AA meetings,
the challenges of living in society today, avoiding drug and alcohol use and avoiding domestic violence incidences.

daily information and 10 client’s
charts on a daily basis for 12 weeks. The charts will note their progress within the program, the overall attitude of the
client, medication compliance, and progress in working towards assigned goals.
62. I will co-lead 10 life skills groups. Topics will include: personal hygiene, interacting
in assertive but not aggressive ways, transportation, anger management, avoiding drug and alcohol triggers, and looking for
work.

will instruct five classes for the service workers international covering changes in procedures.

and English class work daily for nine weeks.

recorded daily information on for students and no daily progress towards assigned individual education plan goals. This will
include behavior, ability to write assignments without redirecting, ability to complete class assignments on their own.

teach class curriculum to four students by rewriting class materials according to each student’s level of comprehension and understanding for nine weeks. The students are disabled and requires class curriculum be broken down and explained to each according to individual level of understanding.

will facilitate 10 addiction group counseling sessions we will discuss maintaining sobriety by studying the big book of Alcoholics
Anonymous.
70. I
will hold individual counseling sessions on drug use and how to avoid situations that lead to drug use, avoiding violence
and personal and family settings, avoiding living situations that may lead to using drugs and alcohol.

will do intake on five clients to assess their drug and alcohol use of choice and their treatment history. Why they are entering
this treatment facility and if they fit the profile for inclusion into our program.

will review records for 20 clients. I will assess completeness, accuracy and compliance with our regulations. If incorrect

will complete an individual assessment on incoming clients which will include questions on substance abuse history, employment
experiences, and family issues.

will assist 10 clients with obtaining one or more of the following ID, Social
Security Disability, tuberculosis testing, work to obtain benefits according to state
income guidelines.

maintain 10 client’s records accurately. I will use appropriate coding and complete
records to document all client activities for 10 weeks.
